falha is defined also as an adjective, which triggers the disambiguation rule DNA (determiner + noun + adjective). I have checked and, in orthodox Portuguese, there is no ‘falha’ as an adjective, so please, add the following to removed.txt.

Falha is an adjective. It’s the female form of falho.
